The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.
Certifications:
- Proof of Auto Insurance
- Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
- Driver's License - Secretary of State (SOS)
- Licensed Physical Therapist (PT) within 90 days - Illinois Department of Financial and Professional Regulation (IDFPR)
Education: Bachelor's Degree in Physical Therapy (Required)
- Document patient visits comprehensively to ensure clarity for subsequent care providers.
- Educate and document discharge planning, including communication with physicians, insurance, outpatient, and community resources.
- Update and evaluate goals regularly based on patient progress.
- Communicate with the care team and document interactions.
- Prepare and supervise staff and assistants in patient homes.
- Administer treatments involving physical agents, and evaluate and fit prosthetic and orthotic devices.
- Plan and administer physical therapy treatments to restore function and prevent disability.
- Perform assessments such as range of motion, gait analysis, and measurements, and document findings.
- Review referrals and medical records to determine the need for therapy.
- Adhere to regulatory requirements and complete assessments, including OASIS and home safety evaluations.
- Communicate with patients regarding scheduling and educate on home exercise programs.
- Perform wound care as per plan of care and department policy.
