Summary
  • The Physical Therapist assumes responsibility for assessing patients' physical abilities, restoring maximum function, maintaining performance, relieving pain, preventing disability or deformity, and developing or improving physical capabilities.
Responsibilities
  • Provides skilled physical therapy to patients in the home.
  • Manages home health cases independently, reviews patient progress biweekly, and communicates with the care team and external agencies as needed.
  • Conducts assessments to identify patient needs, evaluates the home environment, and assists in obtaining medical equipment.
  • Contacts physicians regarding patient status and updates care plans accordingly.
  • Develops and updates individualized care plans based on assessments.
  • Delivers direct skilled therapy, educates patients, and reassesses responses to therapy.
  • Supervises home health aides biweekly, ensuring care plans are followed and updated.
  • Prepares patients and caregivers for discharge, documents all care activities promptly, and schedules visits according to the care plan.
  • Completes all required documentation, participates in team meetings, and adheres to all policies and procedures.
Credentials
  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)
  • Driver's License (Required)
  • Licensed Physical Therapist in Pennsylvania (Required)
Education or Equivalent Experience
  • Bachelor of Arts or Science (Required)
  • 1+ years of home health physical therapy experience preferred.
