Summary:
  • The Physical Therapist assumes responsibility for assessing the physical abilities of patients, restoring maximum function, maintaining maximum performance of patient's capabilities, relieving pain, preventing further disability or deformity, and developing or improving the physical abilities of the patient.
Responsibilities:
  • Provides skilled physical therapy to patients in the home.
  • Provides home health management when a primary nurse is not involved with the patient case. Reviews patient progress with other disciplines involved, at least every 14 days, and communicates with social and/or government agencies providing services to a patient.
  • Assesses and identifies patient needs and problems to determine the required care and services, including physical capabilities, home environment safety, and medical equipment needs.
  • Contacts physicians regarding patient needs, changes, or problems, and reports initial assessment and care plans to the team.
  • Develops and updates a care plan based on assessment findings, prioritizing patient needs and revising as health status changes, at least every 62 days.
  • Delivers direct skilled therapy, reassesses patient response, and revises care plans accordingly.
  • Manages care for Managed Care patients, updating case management departments and following approved visit schedules.
  • Supervises home health aides biweekly, reviewing and updating care plans and ensuring patient/caregiver satisfaction.
  • Discharges patients when skilled care is no longer required, preparing them and notifying relevant parties.
  • Completes timely documentation including assessments, progress notes, discharge summaries, and treatment plans.
  • Schedules patients per treatment plan, including weekend/holiday visits, and discusses treatment plans with physicians for updates or changes.
  • Participates in performance improvement, maintains confidentiality, attends team meetings, and precepts new employees or students.
Credentials:
  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)
  • Driver's License (Required)
  • Licensed Physical Therapist in Pennsylvania (Required)
Education or Equivalent Experience:
  • Bachelor of Arts or Science (Required)
  • 1+ years of home health physical therapy experience preferred.
