Mass General Brigham Home Care is seeking a Physical Therapist to deliver 1:1 home-based care in the Adams team, with a focus on mobility, independence, and safety for patients in their homes. You will work within a collaborative interdisciplinary team to create individualized plans and ensure consistent patient progress in real-world settings.

The role offers hybrid scheduling across Canton, Stoughton, Randolph and surrounding communities, with about 5–6 visits per day and dedicated time for

Qualifications

  • One year experience in acute care, rehab or homecare as a Physical Therapist.
  • Registered with the Massachusetts Board of Registration in Physical Therapy.
  • Ability to prioritize work and adapt to changing situations and travel within the service area.

  • Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes

As a Physical Therapist on our Adams team, you will: • Deliver 1:1 physical therapy care in patients’ homes functioning as a Case Manager • Evaluate patients and develop personalized treatment plans • Focus on mobility, strength, fall prevention, and functional independence • Assess home environments to promote safety and independence • Educate patients and caregivers on recovery strategies and long-term mobility • Collaborate with nurses, therapists, physicians, and social workers to support recovery You won’t just treat symptoms — you’ll help patients regain independence and confidence in their own environment. Primary Coverage Area Most of your time will be spent in Canton, Stoughton, Randolph, while the Adams team also supports the following communities: Avon • Braintree • Holbrook • Mattapan • Milton • Quincy While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible!
