Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery, or injury and manage exacerbations of chronic disease. The Home Care Physical Therapy team focuses on 1:1 patient care at home, mobility, fall prevention, and safety, with a schedule that supports work-life

Qualifications

  • One year of experience in acute care, rehab or homecare as a Physical Therapist
  • Registered with the Board of Registration in Massachusetts.
  • Excellent verbal, written and electronic communications skills
  • Computer proficiency and ability to learn new software
  • Ability to prioritize work and adapt to changing situations
  • Travel required within Mass General Brigham Healthcare at Home's geographic area

Responsibilities

  • Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes
  • Daily documentation is accurate and timely
  • Application of standard treatment procedures
  • Teaching is provided to patients and caregivers

What Makes Home Care Physical Therapy Different?

  • Deliver 1:1 physical therapy care in patients' homes
  • Evaluate patients and develop personalized treatment plans

Primary coverage area is in Lynn and Saugus; however, the entire team also supports Nahant.

Schedule & Workload
  • Part-time (32 hours)
  • Average of 5-6 visits per day
  • Dedicated time for documentation, chart review, visit preparation, and care coordination
