The Clark Health and Wellness Center is a new state-of-the-art facility providing comprehensive health and wellness services under one roof, including urgent care, primary and specialty care, rehab, lab, and imaging services.
As a Physical Therapist in this interdisciplinary team, you will evaluate, plan, implement, document, and educate patients. You will also participate in department initiatives.
Responsibilities
A typical day includes:

• Preparing for evaluations, including obtaining patient history, understanding diagnoses, and researching unfamiliar conditions.
• Conducting tests and measures, synthesizing results to diagnose and plan care.
• Establishing and adjusting treatment plans collaboratively with the team and patient support system.
• Assessing and modifying plans as needed, referring to other professionals when necessary.
• Managing discharge planning and providing patient education.
• Providing functional training, musculoskeletal, and neurological interventions.
• Performing additional assessments like cardiac rehab or splinting as needed.
• Recording patient care data following guidelines, including documentation for reimbursement and communication.
• Completing daily billing and maintaining productivity standards.
• Performing other duties as assigned.

Physical requirements include lifting, pushing, pulling, and standing for specified durations.
Qualifications
Required: Graduation from an APTA-accredited program and a valid Physical Therapist license.
Preferred: Experience (entry-level acceptable), brain injury experience, and computer skills.
