Job summary

Synchrony Rehab is seeking a Physical Therapist to provide a full range of PT services in a collaborative interdisciplinary environment. You will perform assessments, plan and implement treatment, and educate patients and families while maintaining compliant documentation.

Must hold a current state PT license with CPR certification preferred. 0–1 years of experience welcome; opportunities for growth and supportive team environment in Louisville, KY locations.

Qualifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred
• Experience: 0-1 years in physical therapy or related setting

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs
• Develop and implement personalized treatment plans including exercises, manual therapy, and modalities
• Educate patients and families on exercise techniques, posture and body mechanics
• Maintain accurate documentation of patient progress and outcomes per facility policies
• Collaborate with interdisciplinary team to ensure comprehensive patient care
• Other duties as assigned
