Job summary

Synchrony Rehab in Louisville, KY is seeking a Physical Therapist (PT) to provide comprehensive PT services, including evaluation, treatment planning and interventions in an interdisciplinary setting.

You will perform patient assessments, develop individualized plans, educate patients and families, document progress, and collaborate with the care team. A valid PT license and CPR certification are required; 0–1 year of experience is welcomed.

Qualifications

• Degree in Physical Therapy from an accredited program.
• 0–1 years experience in physical therapy.
• Must have and maintain a current state PT licensure; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain up-to-date documentation of patient progress and outcomes per facility policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
