Job summary

Synchrony Rehab seeks a Physical Therapist to provide comprehensive physical therapy services in an interdisciplinary environment. You will assess, plan, and deliver therapeutic interventions aligned with professional standards and patient goals.

The role requires a PT license with a degree from an accredited program. Experience level is 0-1 years, with benefits starting after initial period, and a supportive team focused on patient-centered care.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Active PT license in Indiana or eligible for licensure.
• CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress, plans, and outcomes per policies and regulations.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
