Job summary

Synchrony Rehab is seeking a Physical Therapist (PT) to provide comprehensive physical therapy services in a collaborative setting. You will assess, plan and implement treatment to improve patient function while following professional standards.

Qualifications

• Education: Degree in Physical Therapy from an accredited program.
• Licensure: Current state PT licensure or certification; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics and self-management.
• Maintain accurate documentation of progress and outcomes per facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
