Qualifications

• Bachelor’s degree in Physical Therapy or equivalent.
• Licensed Physical Therapist in the State of Illinois.
• BLS certification with American Heart Association.

Responsibilities

• Maintains professional demeanor, provides accurate information to patients and customers.
