Job summary

LifeSpan Therapy is seeking a licensed Physical Therapist (PT) to assess and treat residents, develop personalized plans, and collaborate with a multidisciplinary team to restore mobility and function. The role emphasizes growth, leadership pathways, and commitment to resident well-being in a family-oriented setting.

You will provide evidence-based interventions, monitor progress, and educate residents and families, with opportunities for advancement and involvement in our leadership

Qualifications

• Must be a licensed Physical Therapist (PT) graduate of an ACOTE-accredited school.
• Current state licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication purposes.

Responsibilities

• Assure PT services facilitate optimal resident outcomes and comply with MDS procedures.
• Assess facility residents for PT needs at admission or as indicated.
• Coordinate admission/discharge to PT with Interdisciplinary Team and nursing staff.
• Develop and implement individualized treatment plans based on resident needs and best practices.
• Perform therapy or supervise a PTA and communicate treatment plans to the team.
• Monitor progress and document outcomes in compliance with regulations.
• Educate residents and families on injury prevention, health maintenance, and therapy techniques.
• Use basic computer skills to manage records and schedule appointments.
• Occasionally travel to other locations for PT coverage if needed.
