Job summary

Qualifications

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, and pain management.
• Monitor patient progress, modify treatment plans as appropriate, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, and discharge recommendations.
• Collaborate with physicians and interdisciplinary teams to coordinate patient care and outcomes.
• Maintain accurate and timely clinical documentation in line with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality standards.
• Treat patients across assigned care settings and age groups as required by the facility.
