Job summary

Total Care Therapy in Toledo, OH is seeking a compassionate Physical Therapist to join a growing team of clinicians. You will evaluate residents, develop individualized treatment plans, and implement interventions to improve mobility, reduce pain, and restore independence.

The role requires licensure, CPR/BLS, a valid driver's license, and experience in geriatrics or related settings. You will supervise PTAs, document in EMRs, and educate residents and staff on home exercise programs and safety.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
