Total Care Therapy in Dayton, OH is seeking a compassionate Physical Therapist to join our growing team. You will evaluate, develop, and implement individualized plans to improve mobility, restore function, reduce pain, and maintain independence for residents.

Qualifications

• Current State Physical Therapist (PT) license.
• CPR/BLS certification required.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait, balance, transfer, and functional mobility interventions.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation using EMR.
• Educate residents, caregivers, and staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload while delivering quality therapy.
