Job summary

Custom Home Health is seeking a licensed Physical Therapist to join our dedicated team in Michigan. You will deliver individualized therapy, helping patients regain independence, improve mobility, and return to daily activities in a home health setting.

Collaborate with RNs and interdisciplinary teams, educate patients and families, and coordinate treatments with compassion and professionalism. Home health experience is preferred; a passion for excellence and patient-centered care is essential.

Qualifications

• Licensed Physical Therapist license required.
• 1+ years of Physical Therapy experience.
• Home health experience preferred.

• Strong collaboration and communication skills.
