Job summary

Custom Home Health is seeking a licensed Physical Therapist to join our Detroit-area team. You will deliver personalized therapy to help patients regain independence and improve quality of life.

Work with RNs and an interdisciplinary team to design and implement care plans, educate patients and families, and coordinate treatments with a compassionate, professional approach.

Qualifications

• Must hold a valid Physical Therapy license.
• 1+ years of PT experience preferred, new grads encouraged.
• Demonstrates commitment to excellence in patient care.
• Collaborates effectively with interdisciplinary teams.

Responsibilities
