Custom Home Health is seeking a dedicated Physical Therapist to join our metro Detroit team. We provide consistent yet flexible schedules, strong patient-focus, and continuous growth opportunities.

As a PT at Custom Home Health, you will deliver personalized care, collaborate with RNs and interdisciplinary teams, educate patients and families, and help patients regain independence and return to daily life. • Home Health experience is required.
