Job summary

BAYADA Home Health Care is seeking a compassionate Home Health Physical Therapist to reliably serve patients in Springfield, MO and surrounding areas. This is a Full-Time position, delivering care in patients' homes to reduce stress and burnout and promote independence.

As a PT, you will perform home visits, assessments, and develop individualized care plans, collaborating with a nursing/therapy team. You will document care using a touchpad tablet and help patients regain function in a home

Qualifications

• Active PT license in good standing.
• One year of clinical experience preferred.
• Home health care experience welcomed, but not required.
• Must be able to travel locally to cases.

Responsibilities

• Visit clients in their own homes to assist recovery from injury, illness, or surgery.
• Perform home assessments, measure mobility and strength, and document findings.
• Develop and modify individualized PT plans with therapists and nursing.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions with the touchpad tablet.

We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in the Springfield, MO and surrounding area. This is a Full-Time position.

What you’ll do:

Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

What you should have:

• Active and current PT license
• One year of clinical experience preferred
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned
