Job summary

Avir at Park in Wichita Falls, TX is seeking a full-time Physical Therapist to join our in-house team. You will assess residents, create personalized rehabilitation plans, and deliver therapies to improve function and comfort.

Our care team collaborates to provide compassionate, person-centered therapy. Texas license in good standing is required; new grads are welcome, with flexible scheduling and strong growth opportunities in a SNF setting.

Qualifications

• Active Texas license in Physical Therapy and in good standing with boards.
• SNF/long-term care experience preferred.
• Knowledge of documentation systems and patient education.
• Ability to manage patients with different personalities.

Responsibilities

• Consult with patients to assess their physical condition.
• Develop and implement individualized rehabilitation plans.
• Administer PT treatments to relieve pain and restore function.
• Ensure documentation complies with care plans and regulations.
• Collaborate with IDT for optimal patient outcomes.
• Provide education to patients and families.

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Complying with all rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
