Job summary

Avir at Moody in Fairfield, TX is seeking a Full-Time Physical Therapist to join our in-house team serving residents with person-centered care.

We offer a collaborative, compassionate environment with a focus on quality outcomes. Texas licensure is required, and SNF/long-term care experience is preferred. New grads are encouraged to apply and we provide competitive compensation and benefits.

Qualifications

• Active Texas license in Physical Therapy and in good standing.
• SNF/long-term care experience preferred.
• Familiarity with Net Health Electronic Documentation System.
• Ability to manage patients with varying personalities.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess evaluations and test results to guide care.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs.
• Administer PT treatments to relieve pain and restore function.
• Ensure treatments align with the established plan of care.
• Monitor patient response and adjust as needed with IDT input.
• Provide clinical education to patients and families.
• Collaborate with the interdisciplinary team to ensure care quality.
• Document thoroughly and timely per regulations.
• Comply with all rules and attend required meetings.

We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

At Avir at Moody, our clinical teams service with compassion and commitment to quality care. Our staff is dedicated to providing customized care options for our residents and collaborate to create these programs and best practices with the intention of exceeding industry standards.

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Complying with all rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
