Job summary

Goodwin Living in Virginia is seeking a dedicated full-time Physical Therapist to join our Home Health team. You will deliver one-on-one patient care at the patient's home, perform comprehensive assessments, and develop individualized plans of care in collaboration with physicians.

You will provide expert therapy per physician directives, partner with a multidisciplinary team to optimize outcomes, and ensure accurate, timely documentation and referrals.

Qualifications

• Graduate of an accredited school of physical therapy.
• Active Physical Therapy License in the Commonwealth of Virginia.
• Prior home health care experience.
• Current CPR certification.
• Possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.

Responsibilities

• Spearhead detailed patient assessments to devise individualized Plans of Care.
• Offer expert physical therapy in the Home Health sphere per physician directives.
• Collaborate closely with the broader team, ensuring optimal patient outcomes.
• Streamline communication with referring physicians, maintaining thorough and timely documentation.

Salary: $96k - $105k DOE
