Description

Summary/Objective:

The Physical Therapist is a licensed therapist who directs, evaluates and provides Physical Therapy services to patients in the clinic or hospital as prescribed by the attending physician, The Physical Therapist is responsible for maintaining standards of professional and ethical practice in the provision of the full scope of Physical Therapy services, while providing the highest quality healthcare with emphasis on the hospitals vision, mission, and values.

Essential Functions:

  1. Provides treatment as ordered by the physician. If a therapist recommends other treatment, calls to get physician approval, and obtains signature of the recommendation in the form of initial evaluation signature or sends MD an order to sign.
  2. Evaluates disability, injury, or disease process of each individual patient and their function.
  3. Interprets, establishes, plans and implements proper treatment programs in accordance with the physician’s referral. Re-evaluate the program every 10 visits or 30 days (whichever comes first) and makes recommendations to the patient and physician as appropriate (to include continuation of the program or discharge) for submittal.
  4. Contact the physician directly for care as needed.
  5. Works in collaboration with the physician and other members of the health professional team in determining the needs of the patients and in developing their total plan of care.
  6. Documents each visit in patients chart.
  7. Observes, records and reports in the chart and patient’s reaction to the therapy program or any changes in the patient’s condition, and reports to the physician as necessary.
  8. Documents nonattendance in patient’s chart.
  9. Input and check patient charges for billing purposes.
  10. Instructs, teaches and supervises patients, their families and other health team personnel regarding physical therapy procedures as related to the patient physical therapy programs.
  11. Evaluates (as necessary) the home environment and makes appropriate recommendations.
  12. Travel to different Physical Therapy clinics that are operated by Sweeny Community Hospital, to include hospital inpatients.
  13. Participates in staff development activities and attends and participates in hospital forums/meetings.
  14. Attend and participate in monthly department meetings.
  15. Participates in scheduled patient case conferences.
  16. Participates in quality improvement by identification of problematic areas and active participation in problem solving process improvement.
  17. Represents the department in a professional and loyal manner at all times.
  18. Respects the right, dignity, privacy and property of others.
  19. Maintains confidentiality of all patients, hospital or physician related information.
  20. Uses available time to department advantage and assist where needed; demonstrates ability to be a self-starter.
  21. Willing to adjust personal schedule to accommodate department scheduling needs.
  22. Completes appropriate number of CCU’s for licensure.
  23. Suggest changes in policies or procedures to improve efficiency
  24. Adheres to all rules, regulations, codes, ethics and guidelines in Federal, State and local laws concerned with the practice of Physical Therapy.
  25. Adheres to all policies and procedures established by Sweeny Community Hospital.
  26. Speaks and presents self in a professional manner.
  27. Assists & completes tasks assigned or delegated by the Director of Physical Therapy.
  28. Ensure evaluations, re-evaluations, and discharge summaries are completed in a timely manner.
  29. Enthusiastically embraces the FIVE pillar ownership to include contributing and helping to achieve departmental and organizational metrics.

Work Environment:

May be exposed to infectious and contagious diseases. May be exposed to the risk of blood born diseases. Occasional exposure to unpleasant patient elements.

Qualifications

Physical Demands:

Frequent bending, squatting, twisting and reaching; occasional patient handling and transferring over 100 pounds; occasional lifting from floor up to 100 pounds. Occasional crawling, climbing, sitting, frequent repetitive pushing, pulling and grasping up to 50 pounds, occasional fine repetitive manipulation; occasional repetitive pushing, pulling and grasping up to 100 pounds.

Position Type/Expected Hours of Work

This is a full-time position, and core hours of work are Monday through Friday 7– 6 , 10 hours per day with no overtime or with overtime as needed as well as one hour for lunch. Position requires work schedule flexibility based on needs of the facility.

TRAVEL

Occasional or frequent travel to and from: Physical Therapy clinics that are operated by Sweeny Community Hospital and Sweeny Community Hospital.

Required Education, Experience and License

Minimum education: Bachelor’s Degree in Physical Therapy from an accredited Physical Therapy Program.

Must have a current license to practice Physical Therapy from the Texas Board of Physical Therapy Examiners.

Current CPR card from American Heart Association

Preferred Education, Experience and License

A minimum or 2 years experience in an outpatient setting preferred.
