Job summary

CARIN Alliance is seeking an outpatient Physical Therapist to evaluate, plan, and implement patient care in line with professional standards and organizational policies.

You will conduct therapy evaluations, develop individualized plans, educate patients and caregivers, assist with discharge planning, and collaborate with a multidisciplinary team to optimize clinical and financial outcomes. Weekend on-call rotation is required, with potential for remote or hybrid work options.

Qualifications

• Current licensure in the state of practice (Minnesota).
• PT degree from an accredited program (Bachelor/Master/Doctorate).

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver evidence-based interventions and document care.
• Provide education to patients and caregivers and assist with discharge planning.
• Coordinate care with a multidisciplinary team and leadership for cohesive patient care.
• Support administrative tasks including documentation, billing, and scheduling.
