Apex Home Health, Hospice & Home Care is seeking a licensed Physical Therapist for outpatient services. You will evaluate, plan and implement therapy for a diverse patient population, coordinating with the care team and educating patients and families.

Role Overview

Provides skilled physical therapy services in an outpatient setting, including evaluation, treatment planning, and implementation for a diverse patient population spanning orthopedic, sports medicine, neurologic, vestibular/balance, oncology rehab, lymphedema, and hand therapy. The role involves establishing treatment plans, educating patients and families, collaborating with healthcare providers, delegating tasks to support staff, and documenting care in accordance with departmental policies.

Qualifications & Skills

• Minimum Bachelor’s Degree in Physical Therapy (PT), with a preference for Master's Degree
• Licensed Physical Therapist
• Open to both new graduates and experienced therapists
• Strong communication, teamwork, and clinical skills
