Job summary

Sage Event Management Inc. is seeking a full-time Physical Therapist (PT) to join our in-house team and support residents in mobility, strength, and quality of life. You will assess, design, and implement individualized rehabilitation plans, administer treatments, and document care in accordance with regulations.

The role emphasizes interdisciplinary collaboration, ongoing patient education, and adherence to care plans with a focus on improving outcomes in a SNF/long-term care setting.

Qualifications

• Active Texas Physical Therapist license in good standing
• SNF/long-term care experience preferred
• Knowledge of current physical therapy practices
• Experience with Net Health Electronic Documentation System preferred

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Physical Therapist license

• In good standing with all regulatory agencies and licensing boards

• Skilled Nursing Facility (SNF) or long-term care experience preferred

• Current knowledge of physical therapy treatment practices

• Experience with Net Health Electronic Documentation System preferred

• Ability to manage patients with varying personalities and needs

• Strong communication, collaboration, and clinical decision-making skills
