Job summary

Honey Grove Nursing Center in Texas is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house team. You will assess patients, develop individualized treatment plans, and work to improve mobility, strength, and quality of life.

The ideal candidate holds an active Texas PT license, has experience in SNF or long-term care, and is proficient with Net Health EDS. We value collaboration, clear communication, and compassionate patient care.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health EDS preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

• Skilled Nursing Facility (SNF) Or Long-term Care Experience Preferred
• Current knowledge of physical therapy treatment practices
• Experience With Net Health Electronic Documentation System Preferred
