Creative Solutions in Healthcare - East is seeking a Full-Time Physical Therapist to join our in-house rehab team. You will provide person-centered care to residents to improve mobility, strength, and overall quality of life.

Responsibilities include conducting assessments, developing individualized plans, administering treatments, and documenting thoroughly. Collaboration with an interdisciplinary team is essential to ensure high-quality care.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches with the care team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)
