Qualifications

Job description

Summary

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps develop a treatment plan, performs interventions aimed at improving patient well-being, and evaluates patient progress.

Hourly Rate: $38.37 - $48.80 (depending on experience)

Responsibilities
  1. Evaluate the patient's functional status, all relevant body systems as required for CMS documentation, and physical therapy needs. Consult with the physician to develop the therapy plan of care.
  2. Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  3. Observe and report the patient's response to treatment and any condition changes to the nurse supervisor and/or physician.
  4. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using accepted standards and evidence-based techniques.
  5. Instruct and inform healthcare team members, patients, and families/caregivers about the plan of care, functional limitations, and progress toward goals.
  6. Visit patients within the service area and nearby agencies as needed.
  7. Complete progress notes for each visit and submit them per policy.
  8. Conduct patient evaluations, develop the PT plan of care within state guidelines, and report outcomes, goals, and projected care frequency within 24 hours of evaluation.
  9. Participate in staff meetings and committees as requested.
  10. Supervise home health aides and PTAs as permitted by policy and state regulation.
  11. Maintain scheduling flexibility, including participation in weekend rotations and observed holidays, to meet patient needs.
  12. Prepare discharge summaries and periodic reports on patient condition every 60 days.
  13. Provide in-service education as needed.
  14. Adhere to LHC Therapy Standards of Practice.
  15. Access ongoing education to stay current with standards of care and maintain licensure.
  16. Perform other duties as assigned.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy license in Colorado.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle.
