Job description

Summary

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps develop a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Salary: $38.37 - $48.80/hour (depending on experience)

Responsibilities

• Evaluate the patient's functional status, body systems, and physical therapy needs; consult with physicians for developing the therapy plan of care.
• Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
• Observe and report patient responses to treatment and any condition changes to supervisors or physicians.
• Treat patients to relieve pain, restore or maintain function, using evidence-based techniques.
• Educate patients, families, and healthcare team members about the plan of care, limitations, and progress.
• Visit patients within the service area and nearby agencies as needed.
• Complete progress notes after each visit and submit them per policy.
• Conduct evaluations and develop PT plans of care within state guidelines; report outcomes and care plans within 24 hours.
• Participate in staff meetings and committees as required.
• Supervise home health aides and PTAs as permitted by policy and regulation.
• Maintain schedule flexibility, including participation in weekend rotations and observed holidays.
• Prepare discharge summaries or condition reports every 60 days.
• Provide in-service education as requested.
• Follow LHC Therapy Standards of Practice.
• Maintain current clinical knowledge and licensure through ongoing education.
• Perform other duties as assigned.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Colorado.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
