Page Health & Rehab Center in Fort Myers, FL is seeking a compassionate Full-Time Physical Therapist to join our in-house subacute rehabilitation team. Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of Florida.
• CPR certification or willingness to obtain upon hire.
• Experience in skilled nursing or subacute rehabilitation preferred but not required.
• Strong communication, organizational, and interpersonal skills.

Responsibilities

• Evaluate residents’ mobility, strength, balance, endurance, and functional abilities.
• Develop and implement individualized physical therapy treatment plans.
• Provide high-quality, evidence-based PT services aligned with patient goals.
• Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and other team members.
• Educate residents, families, and caregivers on treatment plans, mobility techniques, and safety strategies.
• Maintain accurate clinical documentation in compliance with regulations.
• Participate in care planning meetings and discharge planning.
• Adhere to HIPAA and infection control guidelines.

We are currently seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our collaborative rehab team.

A sign-on bonus of up to $10,000 is available for qualified new full-time Physical Therapists.

Physical Therapist Position Summary

The Physical Therapist evaluates residents’ physical abilities, develops individualized treatment plans, and delivers evidence-based therapeutic interventions to restore mobility, strength, balance, and functional independence. This role works closely with an interdisciplinary team to ensure coordinated and effective patient care.

Physical Therapist Key Responsibilities

• Evaluate residents’ mobility, strength, balance, endurance, and functional abilities
• Develop and implement individualized physical therapy treatment plans
• Provide high-quality, evidence-based PT services aligned with patient goals
• Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and other interdisciplinary team members
• Educate residents, families, and caregivers on treatment plans, mobility techniques, and safety strategies
• Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards
• Participate in care planning meetings, interdisciplinary team conferences, and discharge planning
• Adhere to all state, federal, and facility policies, including HIPAA and infection control guidelines

Physical Therapist Qualifications

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license in the state of Florida
• CPR certification (or willingness to obtain upon hire)
• Experience in skilled nursing or subacute rehabilitation preferred, but not required
• Strong communication, organizational, and interpersonal skills
• Ability to work independently and collaboratively within a multidisciplinary team
