Job Description

Summary

We are hiring a Physical Therapist to join our team in Senior Living.

Responsibilities

• Evaluate patients to determine rehab potential, identify needs, develop individualized treatment plans, and set goals within the team plan of care.
• Establish short-term and long-term goals.
• Properly and safely utilize equipment, maintain logs for inspection, maintenance, and cleaning, and report broken or faulty equipment.
• Maintain logs for required outpatient clinic equipment.
• Follow medical regimens and adhere to precautions (e.g., THR, TKR, cardiac, wound care).
• Use safe treatment techniques such as transfers, positioning, feeding, splinting, paraffin baths, cryotherapy, ultrasound, electrical stimulation, etc.
• Complete discharge summaries on the final patient visit to outpatient PT.

Education and Experience

• Current Physical Therapist licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
