Job Description
Summary

We are hiring a Physical Therapist to join our team in Senior Living.

Responsibilities
  • Evaluate patients to determine rehab potential, identify needs, develop individualized treatment plans, and set goals within the team plan of care.
  • Establish initial short-term and long-term goals.
  • Use equipment safely, maintain inspection and maintenance logs, and report any broken or faulty equipment.
  • Maintain logs for outpatient clinic equipment.
  • Follow medical regimens and adhere to precautions (e.g., THR, TKR, cardiac, wound care).
  • Utilize safe treatment techniques such as transfers, positioning, feeding, splinting, paraffin baths, cryotherapy, ultrasound, electrical stimulation, etc.
  • Complete discharge summaries for each patient at their final outpatient visit.
Education and Experience
  • Current Physical Therapist license in the practice state.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, access to reliable transportation, or public transit.
