Job summary

Warsaw Rehabilitation And Healthcare Center in Warsaw, NC, seeks a dedicated Physical Therapist for a full-time role in our long-term care facility. You will evaluate patients per MD orders and develop individualized treatment plans, using a range of modalities while coordinating with an interdisciplinary team.

You will document progress, adjust plans as needed, and participate in scheduling, meetings and staff education.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Develop and implement a comprehensive treatment plan including long and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatment to patients using appropriate modalities and modify as indicated.
• Document daily patient participation and progress on weekly progress notes.
• Prepare and update discharge summaries and in-service materials as required.
• Supervise junior staff or students and co-sign notes as required.

B.S. / Doctorate / MA / MS in Physical Therapy

Current Physical Therapy license
