Job summary

Community Skilled Health Care Centre in Warren, Ohio, is seeking a dedicated Physical Therapist to join our interdisciplinary rehab team. You will help residents regain strength, mobility, and independence so they can enjoy the best quality of life possible.

Current Ohio PT license required; new grads welcome. You’ll collaborate with nursing, OT, and SLP, educate patients and families, document progress, and contribute to a compassionate, patient-centered care environment.

Qualifications

• Current Ohio Physical Therapist license in good standing.
• Experience in a skilled nursing facility or rehab setting preferred, but new grads are welcome to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide direct therapy services in accordance with physician orders.
• Work collaboratively with nursing, occupational therapy, and speech therapy teams.
• Educate patients, families, and caregivers on treatment plans and safe home exercises.
• Accurately document treatments and progress.
