Job summary

Fairview Rehabilitation And Healthcare Center in Franklin, VA is hiring a Physical Therapist for full-time, part-time, or PRN roles. You will evaluate patients, create treatment plans, document progress, and collaborate with an interdisciplinary team to optimize patient outcomes.

Qualifications

• Graduate of an accredited university with a PT degree and licensure in the state of Virginia.
• Current license in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document evaluations, progress notes, and discharge summaries.
• Develop and implement a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Coordinate care with the interdisciplinary rehab team and attend meetings as indicated.
• Schedule patients and manage caseload to meet productivity targets.
• Ensure HIPAA guidelines and facility policies are followed in all documentation.
