Job summary

Fairview Rehabilitation And Healthcare Center in Franklin, VA is hiring an Occupational Therapist to join our in-house rehab team. The role focuses on comprehensive evaluations, treatment planning, and active participation in an interdisciplinary team.

Qualifications include an OT degree and NC licensure with SNF experience preferred. We offer competitive rates, comprehensive benefits, and flexible scheduling within a Long Term Care setting.

Qualifications

• Graduate of an accredited university with an OT degree (B.S./M.S./D.O.) recognized by AOTA.
• SNF experience preferred.
• Licensed and/or registered to practice OT in NC.

Responsibilities

• Perform comprehensive OT evaluations based on physician orders.
• Document findings on standardized forms promptly and accurately.
• Develop a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for all evaluations, treatments, recerts and discharges.
• Provide treatment using modalities and adjust as patient condition requires while observing safety protocols.
• Document daily participation and progress in progress notes.
• Update treatment plans monthly for recertifications and ensure timely discharge summaries.
• Complete caregiver or discharge in-services as required.
• Complete all facility documentation forms as needed.
• Prepare care plans and information for MDS as directed by facility.
• Attend facility and rehab meetings and participate in committees as indicated.
• Serve as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and performing screens as needed.
• Complete billing for OT services accurately and ethically.
• Cover other therapists' caseload during absences when required.
• Supervise junior staff, students, and technicians; co-sign notes where needed.
• Maintain professional relationships with staff, patients, and families.
• Provide in-service education on patient care or OT services to rehab staff and community.
