Saber Healthcare Group is seeking a Physical Therapist for a skilled nursing facility in a PRN capacity. You will work to restore residents to their optimal physical function and guide them toward independent activity.

You will educate patients on ongoing exercise plans, modify activities as needed, and collaborate with a multidisciplinary team to meet therapy goals and ensure high-quality care.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Active, unencumbered PT license in the state of employment.
• Basic computer skills and ability to navigate software including service delivery and EHR.

Responsibilities

• Help patients reach optimal physical function in SNF settings.
• Educate patients to maintain function after discharge with home exercise programs.
• Collaborate with physicians and therapy teams to achieve goals.
