Job summary

Lincoln Meadows Care Center in Lincoln, CA is seeking a dedicated Physical Therapist (PT) for a full-time role in our rehabilitation department. You will evaluate residents, create individualized treatment plans, and help regain independence while collaborating with a supportive multidisciplinary team.

Qualifications

• Active CA Physical Therapist License -or eligible to apply.
• Graduate of an accredited Physical Therapy program
• Familiarity with EHR systems such as Net Health or PointClickCare is a plus
