Responsibilities

• Evaluate and document evaluations of referred residents per facility policies.
• Provide treatment to help residents regain or maximize function.
• Support education for residents and caregivers to maintain function.
• Maintain timely resident reports per regulatory agencies and payers.
• Collaborate with interdisciplinary team and participate in care conferences.
