Responsibilities

• Evaluate and document assessments of referred residents per facility policies.
• Provide treatment to maintain or improve residents' function within physical abilities.
• Recommend adaptive equipment and preventive measures to prevent decline in function.
• Educate residents and caregivers to support maximal independent function.
• Maintain timely resident reports per regulatory requirements.
• Participate in care conferences and rehabilitation planning.
• Contribute to quality assurance programs and infection control.
