Metropolitan Jewish Health System, Inc. is seeking a licensed Physical Therapist to deliver direct home-care services.
You will assess patients, administer therapy, and coordinate care plans with Rehab management while ensuring timely documentation and compliance with procedures. The role requires a BS in Physical Therapy, NYS PT license, at least one year of PT experience (home care preferred), and reliable transportation to cover assigned areas.
Fee-For-Service Rate: $82 per visit
Orientation daily rate: $250 per day

Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home. You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.
In this role, you will be sponsible for direct patient care, providing physical therapy treatment to home care patients. You will be responsible for coordinating plan of care with Rehab management and submitting appropriate paperwork on timely basis and abiding by all agency, state and federal rules, regulations, policies and procedures.
