Job summary

AristaCare Health Services in Linden, NJ seeks a licensed Physical Therapist to deliver resident-focused PT services on-site. You will assess needs, develop and implement treatment plans, and collaborate with an interdisciplinary team to ensure high-quality care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program is required.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill PT services in accordance with CMS expectations.
• Evaluate and plan resident therapy programs with short and long-term goals.
• Screen residents regularly to determine intervention needs.
• Develop treatment plans and communicate findings to residents and team.
• Complete compliant documentation in NetHealth EMR.
