Upstate Caring Partners is seeking a Registered Physical Therapist to evaluate and treat individuals across our programs. The role requires NYS licensure, a driver’s license, and the ability to travel between sites.

You will work with interdisciplinary teams to develop and implement goals and treatment plans. The position includes responsibilities for documentation, collaboration, and ongoing patient education.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Current NYS Physical Therapy Registration/Licensure.
• 1-3 years of related experience preferred.
• A valid NYS Driver's License and ability to travel between sites.

Responsabilités

• Evaluate and assess individuals on an ongoing basis to develop personalized, meaningful treatment goals.
• Implement therapeutic interventions based on assessment findings, helping individuals enhance mobility, function, and quality of life.
• Educate individuals, families, and team members on treatment plans, therapy methods, injury prevention, positioning, range of motion, and more.
• Collaborate closely with interdisciplinary teams including educators and healthcare professionals ensuring alignment with each IPP/IEP.
• Maintain accurate and timely documentation in accordance with program and regulatory requirements.
• Support departmental operations, including equipment needs, adaptation, and orientation of new staff.
• Participate in agency-wide initiatives that advance our mission and enhance the well-being of the people we serve.

$61,880.00 - $70,016 & Sign-on bonus Eligible!

• Full Time $5,000
• Part Time $2,500 *(20 or more hours a week)
