Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. The role offers Flexible PRN assignments, allowing you to choose when you work while delivering compassionate care in residents' homes or assisted living settings.

Applicants must hold a Florida PT license and have a PT degree. New graduates are welcomed to apply, with competitive compensation and clinical autonomy within a supportive team.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
