Job summary

Sobe Innovative Rehab in Palm City, FL is seeking a licensed Physical Therapist to provide hospice therapy in a home setting. You will evaluate and treat residents in their homes or assisted living communities, developing individualized treatment plans and coordinating with physicians and families.

Documentation in the EMR is required to ensure quality care. This PRN role offers flexible scheduling, clinical autonomy, and a supportive team, with competitive compensation and transparent payment

Qualifications

• Active Florida Physical Therapist license required.
• Degree in Physical Therapy from an accredited program (Bachelor's/Master's/Doctorate).
• New graduates and experienced therapists are encouraged to apply.

Responsibilities
