Job summary

Sobe Innovative Rehab in Palm Bay, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting on a PRN basis. This role offers flexible assignments, competitive pay, and the chance to support older adults in maintaining independence and quality of life.

You will evaluate residents in home or assisted living settings, develop individualized treatment plans, collaborate with physicians and families, and document care in the EMR.

Qualifications

• Licensed Physical Therapist in Florida (PT) with current license.
• Degree in Physical Therapy (Bachelor's/Master's/Doctorate) from an accredited program.
• New graduates encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
