Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in a home setting in Kissimmee, FL. You will assess residents, develop personalized plans, and coordinate with the care team to support independence and quality of life for older adults.

PRN schedule offers flexible assignments and competitive compensation. You will work within an autonomous, supportive team and maintain EMR documentation, serving patients in a single, convenient home-based location.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist license required or eligible.
• New graduates and experienced therapists are encouraged to apply.

Responsabilités

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
