Sobe Innovative Rehab in Port Saint Lucie, FL is seeking a licensed Physical Therapist (DPT/PT) to provide hospice services in home or assisted living settings. This PRN role offers flexibility and competitive compensation while allowing you to focus on patient-centered care.

You will evaluate and treat residents, develop individualized plans, collaborate with physicians and families, and maintain EMR documentation.

Qualifications

• Active Florida PT license required.
• Flexible PRN availability preferred.
• Experience in home health or hospice care is a plus.
