Job summary

Sobe Innovative Rehab in Jensen Beach, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide therapy services for Hospice patients in a home setting. This PRN role offers flexible assignments and the opportunity to help older adults maintain independence and quality of life through evidence-based care.

You will evaluate residents, develop and implement individualized plans, collaborate with physicians and families, and maintain accurate EMR documentation.

Qualifications

• Degree in Physical Therapy from an accredited program (PT/Physical Therapist).
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
